  • Please ensure that you have completed all relevant sections of the application form Only applications containing all the information sought will be considered. CVs WILL NOT BE ACCEPTED.
  • Doctors may enclose CVs as additional information but not as an alternative to a completed application form.
  • Canvassing will disqualify.
  • All applicants must provide 2 referees. Relatives must not be used. References will be sought prior to interview. If you do not wish your present employer to be contacted at this stage please indicate in the referees section of the application form.
  • When recording qualifications please refer to the Personnel Specification. This is the document the recruitment panel refers to when shortlisting. You must ensure that all necessary information relevant to the shortlisting criteria is recorded clearly.
  • All sections of the monitoring questionnaire must be completed. The information is treated as strictly private and confidential and is used for statistical purposes only. No panel member will have access to the questionnaire.
  • All applicants should sign and date their application form or agree to the terms if applying electronically.
  • It is the responsibility of the applicant to ensure the completed form is returned by the closing date advertised on the job. Applications received after the closing date and time cannot be accepted.
 

Consent for Vetting Information

Information about and consent to the POCVA check by applicants for posts involving work with children or adults with a learning disability:
The post for which you have applied involves substantial access to children or adults with a learning disability. Before appointing anyone to such a post, it is our policy to ask for a check to be carried out by the Department of Health and Social Services (DHSS) Pre-Employment Consultancy Service. The purpose of the check is to make sure that people are not appointed who might be a risk to children or adults with a learning disability.

The check will tell us whether you have a criminal record or whether the DHSS holds any other information about you which might have a bearing on your suitability. Any information, which we receive, will be treated confidentially and will be discussed with you before we make a final decision. After that decision is made the information will be destroyed.

We will only ask for the check if your application is successful and we are thinking of appointing you. However, you must tell us now if you have ever been convicted of a criminal offence, or cautioned by the police, or bound over. You must include all offences, even minor matters such as motoring offences and 'spent' convictions, that is, things that happened a long time ago. If you leave anything out it may affect your application.

Please complete the Consent for Vetting section when you apply for a job online. This Consent for Vetting section of the online application form also asks you to give your consent to the Pre-Employment Consultancy Service check. Please note that if you do not consent we will not accept your application.

 

Fitness to Practice Declaration

Statement of Policy regarding fitness to practice proceedings by a licensing/regulatory body and relating to criminal investigations in the UK or overseas.

Registration with the General Medical Council or General Dental Council imposes on doctors and dentists the duty to provide a good standard of medical care for, and behave appropriately towards, patients. Employers within the Health and Personal Social Services also have a duty to ensure that patients receive a good standard of medical care and ensure as far as possible the safety of patients. We therefore need to establish if you have been found guilty of a criminal offence, been bound over or cautioned or are currently the subject of proceedings which might lead to a conviction, an order binding you over or a caution, in the UK or any other country.

Applicants for posts in the Health and Personal Social Services are exempt from the Rehabilitation of Offenders ( Northern Ireland ) Order 1978. We expect that application forms will include a declaration for applicants to complete declaring any previous or pending prosecutions or convictions, including those considered “spent” under this Act. Also, it will include a declaration of any cautions or bind-overs.

We also need to establish if you have been the subject of any fitness to practice proceedings in the past, or if any fitness to practice proceedings are being contemplated, by a licensing or regulatory body in the UK or another country and this is also reflected in the declaration.

This information will be treated in confidence and will not debar you from appointment unless the selection panel considers that it renders you unsuitable for appointment. In reaching such a decision we will consider the nature of the conviction/action, how long ago it took place and any other factors which may be relevant.

Failure to disclose a criminal offence, having been bound over or cautioned or that you are currently the subject of criminal proceedings which might lead to a conviction, an order binding you over or a caution, or fitness to practice proceedings undertaken or being undertaken by an appropriate licensing or regulatory body, may disqualify you from appointment, or result in summary dismissal/disciplinary action and referral to the General Medical Council or General Dental Council for consideration if such a discrepancy came to light.

If you would like to discuss what effect any previous convictions, police investigations or fitness to practice proceedings taken or being taken either in the UK or by an overseas licensing or regulatory body might have on your application, you may telephone the Director of Human Resourcesat the Trust for which you are applying, in confidence, for advice.

    • This application form is in PDF format so when you click on the link you will be able to see a copy of the application form
    • You can then print the form by clicking the print icon or choosing File > Print
  • Applying online allows you to fill in your application form on the website without the need for posting the form to the Trust. To apply for jobs online you must first register with HPSSjobs.com The process is shown below:
The first step is registering with HPSSjobs.com. You will receive a password by email which you can use to log in.
For the second step, click on 'My Profile' and log in. You can then fill in your application form with your personal details so that you can apply for numerous jobs.
After fully completing your application form, find the job you want to apply for and click on 'Apply' below the job.
